Check the rules with your regulator
The checklist is general guidance based on published regulator requirements, and it is not legal advice. Rules change, so always confirm the current requirements with the regulator before you file. In North Carolina, for example, the Division of Health Service Regulation home care page links to the 10A NCAC 13J licensure rules and its own initial survey checklist. In Washington, DC, DC Health posts the home care agency application package.
